Avvikelse is of course ordinarily translated as “discrepancy,” or some kind of divergence. I am looking however for a word that represents when something goes wrong, like an accident, or something not working out right, that still would be within the meaning of the word avvikelse.

You could well translate e.g. avvikelserapport by incident report.
Another alternative to the boring (though correct) deviation is nonconformity, which is the term used in quality assurance.
