Source text - English Pre-Op History: Patient presented with Troponin ve chest pain and was admitted to St. Helier Hospital with a diagnosis of NSTEMI. He was transferred to St. George's Hospital for coronary angiography. His cardiac risk factors include hypertension, hyperlipidaemia, being an ex-smoker and with positive family history. He suffers from gastro-esophageal reflux. His coronary angiogram showed severe three-vessel-disease, including a tight left main stem lesion. Echocardiography showed a good left ventricular function and no significant valvular disease.
Procedure: The left radial artery was harvested and was of good quality. The wound was closed in layers. Long sapenous vein was harvested from the left leg. It was also of good quality. The chest was opened and the Lima was harvested. It had good flow. After systemic heparinisation, CPB was established between the right atrium and ascending aorta.
Myocardial protection: Antegrade and retrograde cardioplegia cannulae were inserted. The aorta was cross clamped and warm blood cardioplegia was given initially antegrade and then retrograde. Further doses of tepid cardioplegia were given via both routes after each anastomosis. All anastomoses were performed under the cross clamp.
Translation - Hungarian Kórtörténet: A beteget megemelkedett troponinszinttel és mellkasi fájdalommal szállították kórházba és ST eleváció nélküli myocardiális infarktus (NSTEMI) diagnózissal a St Helier kórházba felvételre került. Koszorúérfestés céljából a St. George’s kórházba került átszállításra. Koszorúérhez kapcsolódó veszélyeztető tényezői a következők: magas vérnyomás, magas koleszterinszint, dohányos múlt, és pozitív családi kórtörténet. A beteg gastro-oesophagealis reflux-ban szenved. Koszorúér-festése súlyos, három ér betegséget mutatott, amelyek közül az egyik egy szűk bal főtörzs szűkület volt. Echokardiográfia jó balkamra-funkciót mutatott a szívbillentyűk szignifikáns betegsége nélkül.
Eljárás: A jó minőségű bal artéria radialis kivételre került. A sebet rétegesen zártuk. A szintén jó minőségű vena saphena magna a bal lábból került kivételre. A mellkast felnyitottuk és a Lima-t kivettük. Vérárama jó volt. Az egész testet érintő heparinizáció után cardio-pulmonalis bypass-t állítottunk fel a jobb pitvar és az aorta ascendens között.
Myocardium-védelem: Antegrád/retrográd kardioplégiás kanül került behelyezésre. Az aorta leszorításra került és meleg véres cardioplegiat először antegrád, majd retrográd módon adtunk. Minden egyes anastomosis utan mindkét módon a beteg további langyos kardioplégiákat kapott. Az összes anastomosis leszorítás mellett történt.
