Source text - Persian (Farsi) كانسر تخمدان در بین بدخيمي هاي ژنيكولوژيك از بيشترين ميزان مرگ و مير برخوردار است. درمان هاي مختلفي براي بيماران مبتلا به كانسر اپيتليال تخمدان با توجه به شرايط بيمار به كار گرفته ميشود كه ميتوان به جراحي، كموتراپي(داخل صفاقي، نئوادجوانت، نگهدارنده) و راديوتراپي اشاره نمود. يكي از روش هاي درمان موضعي در بيماران با كانسر پيشرفته يا پيتليالي تخمداني، كموتراپي داخل صفاقي است. اين روش به عنوان دومين خط درماني در اين بيماران استفاده مي شود. انتخاب رژ يم كموتراپي داخل صفا قي به دلايل ز ير قابل بررس ي و تامل است1) 75 % بيماران سرطان تخمدان بيماري پيشرفته ي داخل پريتوئن دارند . 2) بيماراني كه پاسخ كامل به درمان اوليه كه شامل جراحي و كموتراپي است مي دهند در حدود%30-50 عود خواهند كرد و محل اصل ي عود نيز داخل پر يتوئن است. 3) اكثر بيماران با تومور اپيتليال پيشرفته ي تخمدان پس از جراحي Debulking به كموتراپي پاسخ مي دهند ولي متأسفانه اين پاسخ كوتاه مدت و پيامد كلينيكي اين بيماري ناموفق مي باشد. .3 يكي از روش هاي درمان نگهدارنده در كانسر اپيتليال تخمدان ، كموتراپي داخل صفاقي است . ميتوان علت ارجحيت اين روش را به دلايل ز ير نسبت داد : خصوصيات فيزيولوژيك و آناتومي كپريتوئن ،غلظت موضعي بالاتر، تماس طولاني تر با تومور و سميت كمتر نسبت به درمان سيستميك. 4 بيماراني از اين روش درمان سود خواهند برد كه حجم كم تومور در حد ميكروسكوپي و يا ماكروسكو پيك از نوع جزيي پس از جراحي كموتراپي اوليه داشته باشند. از داروهايي كه جهت درمان داخل صفاقي قابل استفاده هستند مي توان سيسپلاتين، كربوپلاتين، اتوپوزايد، ميتوكسانترون (Mitoxantrone) پاكليتاكسل، توپوتكان و جم سيتابين را نام برد . 3 هدف ما از انجام اين مطالعه تعيين تاثير درمان نگهدارنده با كربوپلاتين داخل صفاقي در درمان كانسرها ي پيشرفته ي اپيتليالي تخمدان است . مهم ترين دلايل ضرورت انجام اين مطالعه ميزان عود بالاي اين سرطان و فقدان رژيم استاندارد نگهدارنده مي باشد.
Translation - English Introduction
Ovarian cancer has the highest mortality rate among gynecologic malignancies. 1&2 Depending on their conditions, patients with epithelial ovarian cancer are treated in different ways including surgery, chemotherapy (intraperitoneal, neoadjuvant, consolidation) and radiotheraphy. Intraperitoneal chemotherapy is an advanced ovarian cancer treatment method applied as the second-line therapy. Selecting intraperitoneal regime is worth considering for the following reasons: 1) %75 percent of those with ovarian cancer suffer from advanced intraperitoneal diseases. 2) %30-50 of patients who fully respond to initial treatment, i.e. surgery and chemotherapy, will suffer a relapse which primarily occurs in the peritoneum. 3) In most cases, after debulking surgery, patients suffering from advanced epithelial ovarian tumor respond to chemotherapy. However, it is unfortunately a short-term response with unsuccessful clinical consequence. 3
Intraperitoneal chemotherapy is a consolidation treatment method for epithelial ovarian cancer. The following factors make this method preferable: physiological and anatomical characteristics of the peritoneum, higher local concentration, longer contact with the tumor and less toxicity in comparison to systemic treatment.4 This method is useful for patients who have microscopic or macroscopic partial tumor size after initial chemotherapy.
Drugs used to treat cancer include cisplatin, carboplatin, etoposide, mitoxantrone, paclitaxel, topotecan, and gemcitabine.3 This study is aimed at finding how consolidation treatment with intraperitoneal carboplatin affects advanced epithelial ovarian cancer. This cancer’s high rate of relapse and the lack of a standard consolidation regime are the most important reasons why such a study seems necessary.
