Este efecto es esencial para estabilizar los alvéolos, y para evitar su colapso al final de la espiración, de modo que se mantenga un intercambio de gases adecuado.
El déficit de surfactante pulmonar, cualquiera que sea su causa, da lugar a una insuficiencia respiratoria grave, que en los niños prematuros se domina síndrome de distrés respiratorio (SDR), el cual es una causa importante de mortalidad y morbilidad agudas y puede originar secuelas respiratorias y neurológicas a largo plazo.
Farmacocinética: XXX permanece fundamentalmente en los pulmones tras su administración intratraqueal; la vida media de la dipalmitoilfosfatidilcolina marcada con C14 es de 67 horas en conejos recién nacidos. En el suero y en órganos distintos del pulmón, solo pueden detectarse trazas de lípidos del surfactante, a las 48 horas de administración.
Datos preclinicos de seguridad: Los estudios de toxicidad aguda realizados en distintos especies por vía intraperitoneal e intratraqueal, no mostraron signos de toxicidad pulmonar o sistémica ni mortalidad. El estudio de toxicidad subaguda por vía intratraqueal en el perro, conejo, y rata (14 días) no mostró efectos clínicos, cambios hematológicos, ni alteraciones microscópicos. Además, XXX no mostró signos de toxicidad en la rata por vía intraperitoneal (4 semanas).
Translation - English The pulmonary surfactant is a mixture of substances, principally phospholipids and specific proteins which cover the internal surface of the alveoli and are capable of lowering the surface tension of the air-liquid interface. This effect is essential in order to stabilize the alveoli and to avoid their collapse at the end of exhalation, in such a way that an adequate exchange of gases is maintained.
The pulmonary surfactant deficit, whatever its cause may be, gives way to a serious respiratory insufficiency which in premature babies is called Respiratory Distress Syndrome (RDS), an important cause of acute mortality and morbidity that can generate long-term respiratory and neurological consequences.
Pharmacokinetics: XXX stays mainly in the lungs after intratracheal administration; the half life of dipalmytoil phosphatidylcholine marked with C14 is 67 hours in newborn rabbits. In the serum and in distinct pulmonary organs, only traces of surfactant lipids were detected 48 hours after administration.
Pre-clinical Safety Data : Acute toxicity studies realized in various species through intraperitoneal and intratracheal tubes showed no signs of pulmonary or systemic toxicity nor mortality. The subacute toxicity studies through intratracheal tubes in dogs, rabbits, and rats (14 days) showed no clinical effects, hematological changes, or macroscopic alterations. Moreover, XXX showed no signs of toxicity in rats when administered through an intraperitoneal tube (4 weeks).
