Scappano da una vita connessa a cavi e sonde, dal ***groviglio ospedaliero***, dalle fauci del forno crematorio, seguendo nell’oscurità un piano verso la leggendaria fonte dell’eterna giovinezza."

Describing a play. Tangle of hospital corridors? or something more figurative: the processes, succession of injections, pills, visits, hours, examinations?

I think you got it right and it could be both ways; besides, a hospital is not only a place for medical treatment, but could also become a bureaucratic nightmare.

complex maze of the hospital

I don't think that "tangle" works, but I do not think that "maze" works on its own here. Therefore I think focussing on a suitable adjective can convey the same image in our language, be it the buildings, the corridors, the ward layout or whatever.
