Ich bin der Unterzeichner dieser eidesstattlichen Erklärung
or something like this. To affirm is in german "versichern beteuern" and affidavit is in german "Eidesstattliche Erklärung".

| Thanks, Michael, "Unterzeichner" would seem to fit the ticket. My problem was with "affirmant", as I don't usually do this kind of translation and I couldn't find it in any of my dictionaries. |
