NHS 信託と NHS 基金の二種があった病院の運営主体は、自律性と独立性の高い NHS 基金に一本化が図られてきた。法はすべての病院に病院監督機構の監督を受けさ せるため、NHS 基金による収益事業等その運営に関する規制を緩和することにより、 NHS 信託の組織変更、合併等による NHS 基金への移行を促進することとした。また、 NHS 信託から NHS 基金への移行に期限を設けたが、財務状況が不安定で移行の見込 みが立たない NHS 信託が確認されたため、期限の設定は事実上撤廃されている(注 4)。
