Source text - English Amyotrophic lateral sclerosis (ALS), also called Lou Gehrig's disease, is a progressive neurological disease affecting 30,000 Americans with about 5,000 new cases occurring in the United States each year.
ALS belongs to a class of disorders known as motor neuron diseases. Motor neurons are nerve cells located in the brain, brainstem, and spinal cord that serve as controlling units and vital communication links between the nervous system and the voluntary muscles of the body. The loss of these cells causes the muscles under their control to weaken and waste away, leading to paralysis. It is usually fatal within five years of diagnosis.

Source text - English 由于经验的积累和新技术、新器械的出现，PCI的适应证在不断扩展，过去认为困难或风险很大的病变现在已经可以很安全地进行治疗，因此难以界定绝对的适应证和禁忌证。
确定适应证和禁忌证实际上是平衡PCI的收益和风险，收益>风险是相对适应证，反之就是相对禁忌证。平衡收益和风险需要考虑很多因素：(1)患者的全身情况能否耐受操作；(2)心肌缺血的严重程度；(3)手术操作成功的可能性；(4)处理并发症的能力；(5)远期效果；(6)费用。临床医师需要与患者本人和家属客观和认真地讨论PCI、外科手术和药物治疗的利弊，在讨论过程中要尊重患者本人的意愿和选择。
在本指南中，适应证主要根据患者的临床症状、心肌缺血的客观证据、PCI成功的把握性、左心室功能及是否合并其他疾病而定。PCI的主要作用是缓解心绞痛，在某些患者和病变可改善预后。一般而言，患者没有症状不应作PCI。某些患者症状虽不严重，但负荷试验显示广泛心肌缺血，患者平常活动量较大，进行血管重建治疗有可能预防致死性心脏事件的发生。

Translation - Chinese With the accumulated experience and emergence of new technologies and devices, the indications of PTCA have been continuously expanded. The diseases having been considered to be refractory or at high risk can be treated in a very safe way now. Accordingly, it is difficult to define the absolute indications and contraindications.
In fact, you are weighing benefits against risks of PCI when you try to identify the indications and contraindications. It is a relative indication when benefits are greater than risks; on the contrary, it is a contraindication. There are many factors you need to consider when you weigh against benefits: (1) whether systemic conditions of a patient can tolerate the procedure ;( 2) severity of myocardial ischemia; (3) likelihood of success; (4) the ability to manage complications; (5) long-term effect and (6) costs. Clinicians need to facilitate a serious and objective discussion with patients and their families about the benefits and risks of PCI, surgery and drug therapies. During the discussion, the willingness and choice of patients must be respected.
In the present guidelines, the indications are identified mainly on the basis of patient clinical symptoms, objective evidence of myocardial ischemia, likelihood of PCI success, functions of the left ventricle and whether it can be accompanied by other diseases. The major effect of PCI is to alleviate angina, and to improve prognosis for some patients and lesions. Normally, PCI should not be performed on the patients with no symptoms. Some patients do not have serious symptoms, but the load test showed an extensive myocardial ischemia. They have many activities in daily life. The revascularization may prevent these patients from occurring fatal cardiac events.

A. Install pumps with access for periodic maintenance including removal of motors, impellers, couplings, and accessories.
B. Independently support pumps and piping so weight of piping is not supported by pumps and weight of pumps is not supported by piping.
C. Install continuous-thread hanger rods and vibration isolation of sufficient size to support pump weight. Fabricate brackets or supports as required.
D. Set base-mounted pumps on concrete foundation. Disconnect coupling before setting. Do not reconnect couplings until alignment procedure is complete.
E. Support pump baseplate on rectangular metal blocks and shims, or on metal wedges with small taper, at points near foundation bolts to provide a gap of 19 to 38 mm (3/4 to 1-1/2 inches) between pump base and foundation for grouting.
F. Adjust metal supports or wedges until pump and driver shafts are level. Check coupling faces and suction and discharge flanges of pump to verify that they are level and plumb.
