Introduction
B-type Natriuretic Peptide (BNP) plays an important role in the pathophysiology of congestive heart failure (CHF).BNP is a counter-regulatory hormone to the reninangiotensin-aldosterone (RAA)cascade and is a potent preload and afterload reducer. As such, it is released in direct response to ventricular wall stress and promotes fluid and electrolyte homeostasis by binding to natriuretic peptide receptor sites.1 BNP can be measured using a rapid, portable whole blood test. Recently, another marker, NT-proBNP, has become available as a diagnostic tool for heart failure. Both BNP and NT-proBNP are cleaved from proBNP. However, NT-proBNP is biologically inactive and does not exhibit the hormonal properties associated with BNP. Unlike BNP, NTproBNP is predominantly cleared through the kidneys, so its utility as a heart failure tool may be limited in patients with renal impairment. In addition, due to its longer half-life (60-120 minutes vs 20 minutes for BNP1),NT-proBNP may be less accurate in reflecting current ventricular status in emergency departments and acute care environments.

El péptido natriurético de tipo B (BNP) desempeña una función importante en la fisiopatología de la insuficiencia cardíaca congestiva (ICC). Es una hormona regulatoria que actúa en contra de la cascada de la renina-angiotensina-aldosterona (RAA) y es un potente reductor de la precarga y de la poscarga. Como tal, se libera directamente como respuesta a la tensión que sufre la pared ventricular, y favorece la hemostasia hidroelectrolítica, al fijarse a los sitios receptores del péptido natriurético.1 El BNP puede determinarse por medio de un análisis de sangre rápido y portátil. Se ha lanzado, hace poco tiempo, otro marcador, el NT-proBNP, como ayuda para el diagnóstico de la insuficiencia cardíaca. Tanto el BNP como el NT-proBNP se producen por la descomposición del proBNP. Sin embargo, el NT-proBNP es biológicamente inactivo y no presenta las propiedades hormonales asociadas al BNP. A diferencia del BNP, el NT-proBNP se depura predominantemente por los riñones, de modo que en los pacientes con insuficiencia renal su utilidad para el diagnóstico de la insuficiencia cardíaca puede ser limitada. Además, debido a su semivida más prolongada (60 a 120 minutos en comparación con 20 minutos en el caso del BNP1), el NT-proBNP puede ser menos exacto para reflejar el estado ventricular del paciente en el servicio de urgencia y en la unidad de cuidados críticos.
