This is the German standard and exactly what you're looking for. As far as I know (as a non-lawyer giving his personal opinion), it's fine in general, although you'll want to personalize a few points and maybe improve a few others.
If you aren't working out of Germany, you may also want to quickly explain the main differences between the relevant laws of your place of business and those in Germany. However, that is probably a huge task and is of dubious worth; I would suggest postponing it until shortly after your death (at the earliest).

That's how I created the terms and conditions for a translation agency here in England.
Change the wording a little bit and noone will notice it.
A good solicitor does nothing different: we were trained to copy, paste and change our drafts all the time!
The successful solicitors have loads of sample documents they rearrange all the time!
