I think the clue is in "sowie weiterhin" - in other words phenprocoumon was discontinued at some point in the past and that discontinuation (and substituted with nadroparin).
Solutions? How about "suspension of Falithrom treatment (in favour of Nardoparin) was continued". Or "The patient was kept of Falithrom but Nadroparin was used instead".

: Phenprocoumon, to make sure that the reader will understand ... and Falithrom, if this is the tradename understood in every English-speaking country. IMO Marcumar would not be understood. 'weiterhin' may just mean 'beside the Antibiotikatherapie'.
