@Judith This is the way it's being put in Russian to determine if the patient makes full payment in cash or uses any form of insurance. I still use a couple of private Russian clinics, so I know:-) They just keep one field for organizations and individuals, and Категория is intended for individuals. They ask me "Are you a cash customer"? (this is my category). I would assume that if the organization is to be entered, then they have a contract with them or know (ask), which insurance company that organisation uses.

payment in cash

Imho, this is the field where some information about the payment should be provided, such as bank name and address, account number etc. If the payment is done in cash, then all this info is not needed.

covered by insurance

My wild guess is that it's not about cash or credit, but rather about the insurance coverage. For example, in a private clinic, you would see a note saying that if certain services are not covered by your insurance, you could receive them on a commercial basis.
