I'd suggest departing a little from the source text to make this sound more English. I've seen this type of phrase on official docs in the UK and there are many GHits for it/variations of it.
or "...is accurate and true to the best of my knowledge"
I suggest adding "I hereby confirm/certify..." because there is a signature/date below it.
