Efficacy Observation of Applying Aescuven Forte to Treat Chronic Venous Insufficiency in Lower Limbs

[ABSTRACT] AIM To observe the efficacy of aescuven forte in treating chronic venous insufficiency in lower limbs. METHOD Divide patients with chronic venous insufficiency in lower limbs into different levels and groups and observe efficacy comparison between drugs and leg compression stocking. RESULT There is definite efficacy of aescuven forte on chronic venous insufficiency in lower limbs. CONCLUSION Aescuven forte functions for CVI patients in early and medium stages with the same effect as leg compression stocking does and works to a certain xf degree for CVI patients in later stage.

Chronic venous insufficiency (CVI) and skin dystrophia are the common diseases in vascular surgery, incidence being around 15%~20%[1], mainly featuring shallow varicose veins, edema in lower limbs and ulcer in skin, which seriously influences patients’ life. Wearing leg compression stocking is thought to be the standard treatment with definite efficacy [2]. Aescuven forte is the extract of horse-chestnut seeds and has been used to treat CVI in Europe for almost 20 years; it mainly functions to inhibit inflammatory reactions, improve permeability in vessels, promote venous and lymphatic return as well as controlling and delaying the development of CVI. Randomized control studies found aescuven forte safe and effective [3]. We conducted control study on 77 cases with CVI from March, 2001 to December, 2003 by using aescuven forte and leg compression stocking in treatment with the following report.
1 Clinical data
1.1 General data: we randomly selected 77 CVI cases diagnosed by venous radiography and ultrasonic Doppler, among whom were 42 male cases and 35 female cases (age ranged from 18~77 and averaged 51.06±13.40). They suffered from 1~42 years of CVI, averaged 16.55±11.60. 77 cases showed shallow varicose veins clinically; 52 cases showed distending pain in shanks. 77 cases showed edema. 54 cases showed pigment sediment in feet and shanks while 24 cases showed venous ulcer.
1.2 Method: Conduct pre-treatment evaluation on selected patients for clinical symptoms and signs, subjective symptoms including pains, swelling, heaviness and spasm in mild, moderate and severe types while conducting CVI clinical grading. Clinical grading standards[4] include the following: level 0, invisible or untouchable venous disease; level 1, angiotelectasis and reticular vein and swelling ankle; level 2, shallow varicose vein; level 3, edema without change in skin; level 4, edema and venous diseases that caused changes in the skin (pigment sediment, eczema, lipodermatosclerosis); level 5, skin changes along with recovered ulcer; level 6, skin changes along with active ulcer. Patients with levels 1, 2 and 6 didn’t receive clinical efficacy evaluation for CVI. Among the patients with CVI symptoms in lower limbs, the ones also with severe edema in lower limbs, thrombotic phlebitis, formation of venous thrombosis, arterial diseases and diabetes weren’t chosen. Clinical grading of CVI patients: aescuven forte group has 15 cases of level 3, 17 cases of level 4 and 9 cases of level 5; leg compression stocking has 8 cases of level 3, 14 cases of level 4 and 15 cases of level 5.
Study group: aescuren forte produced by Cesra pharmacy in Germany (1tablet/time, 2 times/d) were given; as for the control group, Venosan produced in Germany was given to the patients by their figure to ensure 20~30mmHg pressure in lower limbs and patients wore the stocking everyday. We conducted repeated measurement and record of subjective symptoms and perimeter of shanks by the end of 2nd, 4th and 6th week after treatment and conducted statistical analysis at last.
2 Result
The group receiving drug treatment had similar efficacy with that in the group using leg compression stocking. In the 6th week, there was no significant difference (P>0.05) in improvement rate of shanks’ perimeter and subjective symptoms in both groups.
