has been registered for follow-up
I've found this an efficient way to express the idea of "учет" in English, rather than writing out something like "entered into the records for follow-up." If the rest of the context does not already make this clear, you can add "medical" or substitute "continuing check-ups."

: Please check this link: http://tinyurl.com/zptho2x / It is the same as in my answer. It was discussed previously.

I read the 2008 discussion. None of the suggestions there combined "registered" with "follow-up" or "continuing check-ups," which I believe is the best combo, and which I took from my own TM, not from that KudoZ discussion.
