Вынуждены Вам сообщить, что сроки устранения неполадок, заявленные Вами в предоставленном графике работ, в очередной раз сорваны.

Бьюсь над формулировкой - please advise!
Смысл в том, что поставщик оборудования обязался устранить выявленные неполадки и для этой цели составил график работ по их устранению, однако в него не уложился.
