Source text - Spanish La hepatitis B crónica (HBC) es una enfermedad
devastadora que puede evolucionar a cirrosis, insuficiencia
hepática y cáncer de hígado.
La instauración de un tratamiento efectivo resultará
en menores posibilidades de desarrollo de fallo
hepático. En la actualidad, el tratamiento con análogos
nucleos(t)ídicos (AN) está autorizado para un
manejo seguro de esta enfermedad. Sin embargo, el
hepatocarcinoma (HCC) continúa siendo el 5º cáncer
más frecuente, representando alrededor del 5%
de todos los cánceres.
Está demostrado que la terapia con interferon (IFN)
reduce el riesgo de desarrollar HCC, mientras que
hasta el momento no hay datos que indiquen lo
mismo con los AN.
Los principales marcadores subrogantes de respuesta
sostenida al tratamiento son: la seroconversión
del HBeAg y el clearance del HBsAg.
La aparición de los anticuerpos contra estos dos
antígenos está asociada con una reducción del
riesgo de desarrollar cirrosis y HCC, y con una
mejoría en la supervivencia. El riesgo de desarrollar
HCC es menor si el clearance del HBsAg
ocurre antes de los 50 años de edad (Yuen MF, et
al. Gastroenterology 2009).
Todas las guías de manejo de la hepatitis B (HVB),
por ejemplo, las de AASLD, EASL y APASL, coinciden
en remarcar la importancia del clearance del HBsAg,
que cumple con un rol clave en la historia natural de
la infección.
Las guías de EASL (J Hepatol 2009; 50: 227-242) indican
que el clearance del HBsAg “…se asocia con
una completa y definitiva remisión de la actividad de
la HBC y con una mejoría en los resultados a largo
plazo”.
Analizando la historia natural de la HBC se puede
observar que el primer estadio consiste en la presencia
de HBeAg, junto con una elevada carga viral
(ADN HVB) y bajos niveles de transaminasas.
Esta fase se define como de inmunotolerancia, y la
sucede la de inmuno–clearance, caracterizada por
una caída de los niveles de ADN HVB, aumento de
las transaminasas y persistencia del HBeAg.
La enfermedad continúa con la fase de inmunocontrol,
en la que la enfermedad se encuentra inactiva:
el HBeAg se habrá negativizado paralelamente con
la aparición del anti-HBeAg y la caída de la carga
viral y de las transaminasas. Según la evolución,
puede sobrevenir una fase de reactivación, con
aumento de carga viral y transaminasas y HBeAg
negativo, o una etapa de clearance del HBsAg. Lo
que se intenta lograr con el tratamiento es que la
enfermedad se torne inactiva.
El objetivo a lograr en el paciente bajo tratamiento
es la declinación de los niveles de HBsAg y la supresión
de ADN HVB. De ese modo se puede aspirar a
alcanzar el control inmune sostenido, definido como
la presencia post-tratamiento de la seroconversión
de HBeAg en sujetos que eran HBeAg positivos, o un
nivel de ADN HVB menor o igual a 10.000 copias/ml en
los pacientes que eran HBeAg negativos. (Cuadro 1)
Translation - English Chronic hepatitis B (CHB) is a devastating disease that can lead to cirrhosis, hepatic failure and liver cancer.
The introduction of effective treatment will result in less chance of developing liver failure. At the present, treatment with nucleoside analogues (NA) is authorized for safe handling of this disease. However, the hepatocellular carcinoma (HCC) is still the 5th most common cancer, representing about 5% of all cancers.
There is evidence that therapy with interferon (IFN) reduces the risk of developing HCC, whereas so far, no evidence to suggest the same thing with NA.
The main surrogate markers of sustained response to treatment are: HBeAg seroconversion and HBsAg clearance.
The appearance of antibodies against these two antigens is associated with reduced risk of developing cirrhosis and HCC, with important improved survival. The risk of developing HCC is less, if the clearance of HBsAg occurs before 50 years of age (Yuen MF, et al. Gastroenterology 2009).
All management guides of hepatitis B (HBV), for example, those from AASLD, EASL and APASL, agree in stating the importance of clearance of HBsAg, which fulfills a key role in the natural history of infection.
The EASL guidelines (J Hepatol 2009, 50: 227-242) indicate that the clearance of HBsAg "... is associated with a complete and definitive remission of the CHB activity and an improvement in long-term results."
Analyzing the natural history of CHB it is possible to see, that the first stage is the presence of HBeAg, along with a high viral load (HBV DNA) and low levels of transaminases.
This phase is defined as immune tolerance, and is follow by immune-clearance, characterized by a fall in HBV DNA levels, elevated transaminases and HBeAg persistence.
The disease continues inmunocontrol phase, in which the disease is inactive: the HBeAg become negative parallel with the appearance of anti-HBeAg and fall in viral load and transaminases. According to evolution, a reactivation phase may occur, with increased viral load, transaminases and HBeAg negative or a clearance of HBsAg stage. What we are trying to achieve with treatment, is that the disease becomes inactive.
Individualized care
for patients with chronic Hepatitis B
The goal to achieve in the patient undergoing treatment is the turn down in the levels of HBsAg and HBV DNA suppression. In this way, we can expect to achieve a sustained immune control, defined as the presence of post-treatment of HBeAg seroconversion in subjects who were HBeAg positive or HBV DNA level less than or equal to 10,000 copies / ml, in patients who were HBeAg negative. (Figure 1).

Source text - English Each year, two million people – mostly children –
die from water-borne diseases such as diarrhoea
and cholera, according to the United Nations.
Particularly vulnerable are those affected by
natural disasters, when gaining access to clean
water can be a problem. However, a new technique
that produces drinking water from seawater using
just small amounts of energy could help to address
this dire situation.
The technique, developed by researchers at the
Massachusetts Institute of Technology (MIT) in the
US and Pohang University of Science and
Technology in Korea, manages to desalinate water
using a simple electronic system. The process
starts by passing water along a tiny, 500 μm wide
channel on a polymer chip. When the water
reaches a junction, it splits off into two separate
tubes. By applying an electric potential along one
of these tubes, salt ions are dragged towards this
channel in the form of brine, while desalinated
water flows down the second channel under the
force of gravity. The researchers have successfully
used the technique, which is dubbed ion
concentration polarization (ICP), to convert
seawater, with a salinity of 30 000 mg l–1, into
fresh water with a salinity of less than 600 mg l–1,
which meets the international standards for water
purity (Nature Nanotech. 5 297).
In terms of energy consumption, ICP compares
favourably with established methods of
desalination, requiring less than 3.5 Wh l–1.
Reverse osmosis, for example, which works by
forcing seawater through a membrane at high
pressures to capture the salt, requires 10–
15 Wh l–1. And electrodialysis, which works by
transporting salt ions from one solution to another
by means of ion-exchange membranes, requires
5 Wh l–1. Another advantage of ICP is that it can
remove other potentially harmful larger molecules
– such as cells, viruses and bacteria – without the
filter becoming heavily clogged, a problem that
affects both reverse osmosis and electrodialysis.
The next challenge is to scale up the device into
a viable technology. As one chip produces just
10 μl per minute, the researchers estimate that
they will need 10 000 combined units to produce a
useful amount of water in a realistic time A device
this size would still be portable at just 30 20 cm.
Sung Jae Kim, one of the researchers at MIT,
told Physics World that his team hope to have
produced a 100-unit device within two years.
One outstanding challenge is to ensure that all
dangerous hydrocarbons and heavy metals are
also removed from the seawater, which is not
guaranteed with the current device.

Otra ventaja de la PCI es que puede quitar otras moléculas más grandes, potencialmente dañinas –tales como células, virus, y bacterias- sin que el filtro se cubra profusamente- , un problema que afecta tanto a la osmosis inversa como a la electrodiálisis.
El próximo reto es aumentar de tamaño el dispositivo para que se convierta en una tecnología viable. Ya que un solo circuito integrado produce solo 10 μl por minuto, los investigadores estiman que necesitarían 10 000 unidades combinadas para producir una cantidad utilizable de agua en un tiempo adecuado. Un dispositivo de ese tamaño seguiría siendo portable midiendo alrededor de 30 20 cm. Sung Jae Kim, uno de los investigadores de MIT, mencionó a Physics World, que este equipo espera producir unos 100 dispositivos en dos años. in Paraguay (Customs)
The purpose of this feasibility study is to explore ways to improve the Paraguay Customs (Dirección Nacional de Aduanas) system and consider how these specific systems fit into the overall Paraguay e-Government plan. This study also addresses what is required to align Paraguay e-border systems with the latest international standards and regulations. The current Paraguay Customs system (Sofia) provides the fundamental functions required for daily operations. While Sofia has laid a solid foundation for electronic customs processing, Paraguay it stills needs more centralization, expansion of electronic data exchange, continued reduction in paper documents, and advanced features, integrated into the existing system.
This study provides a current analysis of Customs systems from both business and technical perspective. It also provides detailed scope of development for ‘future models’. ‘Future models’ and this study include consideration of current international demands and trends in customs and incorporation of international standards as part of the Paraguay Modernization of e-Border System Establishment Plan. All proposed systems will be maintained, supported, and operationally managed by Paraguay government employees enabling future modifications to be made by domestic (Paraguay government) resources and manpower without direct assistance of foreign companies.

Source text - English The temperature gradient feature of the CFX96 real-time PCR system was used to determine the optimal RT temperature for the four-target multiplex RT-qPCR assay. The CFX96 system was programmed to perform RT at temperatures ranging from 45 to 55oC, followed by a three-step PCR protocol (denaturation-annealing-extension). Results from the RT gradient in Table 2 show that the optimal RT temperature range (in blue) was between 45.0 and 51.3 oC for all primer-probe sets, with Flu A reactions working as high as 53.3oC. Selecting higher RT temperatures improves the specificity of the assay but compromises the assay sensitivity. In order to maintain both specificity and sensitivity, we have selected 50oC RT for the rest of the four-target multiplex assay validation.
