clerk's fee
The ______, as Payor, shall pay to the ______, as PAYEE, child support in the amount and method as set forth in Attachment “A”, Payment Information Sheet, attached hereto and incorporated herein. The PAYOR shall pay the **Clerk’s fee** in addition to the payments ordered.
The specific kind and type of child support shall be described in this Final Judgment of Dissolution of Marriage and shall control over general descriptions of child support contained on the attached Payment Information Sheet.
The PAYOR and PAYEE shall notify the Central Governmental Depository within (5) days of any change in the addresses shown on the Payment Information Sheet.
