And it's curious that many people either refuse to give you more context or even get mad when you ask for it. All we can do is smile and go back to work. More context needed Mario, that's what I thought at first, but apparently there was information we didn't know about. Muriel, there has been hardly any context except what we've been told by the Asker. I'd like to see the original text, hoping for some new insights. Context is everything, IMHO. There's nothing like getting to the source.

@ Mario But my point is that the Portuguese was incorrect to begin with, so for this context I don't think the correct translation applies. My solution would probably work best for the asker, but not for the glossary.

De referência not por referência IMO, "de referência" means primary indeed. It's the first doctor you'd resource to, not someone referred by someone else, so not referral, Muriel.

Jacky With the additional clarifications from you, it sounds like PCP may not be the proper translation. I think that direct quotes from the interview might shed some light. Can you post a few excerpts in which the term appears?

@Jacky I am not sure that centro de referência can be translated as reference centre. A reference centre is a documentation and information hub.

A centro de referência is more likely to be medical specialist centre, which is a not uncommon term.

@Jacky The "referring physician" is the the doctor at the lower level, whether primary or secondary. A "physician referral" is a request to be referred up the ladder to a higher level. If you could give us more context, we could give you an accurate answer.

More context We still don't know which end of the referral process the phrase is referring to. It would help a lot to see more context.

Thank you all - I agree with Mark Robertson, and Ana Vozone, and I think this is backed up by the fact that all the physicians the patient is discussing are all experts with extensive knowledge of the rare diseases they are working in and furthermore they work in what are referred to as "centros de referencia", which I understand in English can be called "reference centres". But of course you can't say "reference physician", hence my question.

@All From my research, I gathered that a "médico de referênica" is not a specialist. He/she may refer to a specialist if needed. Here in the US, he/she is often referred as a PCP (primary care physician). Ele / ela "é capacitado para monitorar as pessoas em diversas situações, seja qual for a condição apresentada. I think it's about referral "TO"-- not "FROM" The Portuguese is confusing. Ordinarily, "médico de referência" is the 'referring physician'. However, based on the asker's explanation, the patient has yet to be referred and is being told to seek a physician referral. (That's why I gave my answer a low level of confidence--the context is unclear. I'm confident of the English terminology in both directions.)
